Before procedure

1. We may ask you not to eat 8 hours before your procedure

2. You would need to stop taking all blood thinners, which we ask that you please talk to your

physician before doing so. if you're taking aspirin stop 3 days before, Plavix 8 days before or

Coumadin 5 days before. We may ask you to take a blood test the day of your procedure.

3. You will need to bring a responsible person with you that will stay during your procedure.

4. Do not take any pain medication if you are having a median branch block procedure.

5. Please wear loose comfortable clothing. You might be given a gown to wear once you are

being prepared for the procedure.

6. Please do not bring any valuables, jewelry or money to the clinic. We will not be responsible

for such items.

7. If you begin antibiotics before procedure please call office to make sure we will still be able

to perform procedure

8. $30.00 will be charged if you NO SHOW or if you don't cancel within 24 hours of your

scheduled appointment. NO EXCEPTIONS!
