I regret to inform you that FIRST INTERNAL MEDICINE office stopped seeing patients on November 28, 2019. I encourage all former patients to find a new primary care physician promptly if they have not done so already.
Dr Angelique Poturalski has agreed to accept all First Internal Medicine patients. If you are looking for a doctor whose services and approach to patient care are similar to my own, I encourage you to join her MDVIP-affiliated medical practice.

You can return either form via mail to the office address or by fax (720) 285-1951. You can also scan the signed form and email it to denvermd(at)denvermd.net. Once we receive the signed authorization, the records company will send a copy of your medical records to your new physician promptly.
