About Our Office

Hartley Health and Wellness Associate , LLC provide patients with comprehensive evaluations of all body systems and create a plan care based on individual needs. Our practice follows a patient centered model of care with a personalized reievew of the patient's medical, social, and spiritual needs. We also provide short-term services, which is available upon request by the patient's regular physician. Our goal is to provide timeless primary care with a personal touch to each of our patients.
