If you are a new patient let us first say thank you for choosing Patient First, INC and allowing us to serve you with your healthcare needs. We understand there are many options out there and we look forward to trying to earn the business and trust you have given us.
As a new patient there are a few things we need to do to make the process as quick and easy as possible. We ask that if possible you setup an online account and enter the information requested into your patient portal; this will help streamline your check-in and visit on your visit day. We also request you report to your appointment 15 minuets prior to your schedule appointment time.
We ask that you request any medical records you feel relevant from another provider be forwarded to our office before your visit or brought by you on the day of your visit.
We ask that you please bring the medication bottles for any medication you are currently taking with you at the time of your visit.
We ask that you do not eat or drink anything starting at midnight the night before your exam, with the exception of a few sips of water to take your morning medicine, in case we need to order labs for you that day.
We ask that you understand that if you are transferring to us from another provider we will try our best to ensure that there is a seamless transition and that we don't make several changes, however, we ask you to keep in mind that every provider is different and we make changes depending on our knowledge, experience, current guidelines, and how we feel we can best serve you.
We ask that you take your time, express any health problems or concerns that you may have with us, and to be truthful with us. Often times denial is one of the first symptoms of a big problem. No problem is to big or small for us to address; we have the time. Our goal is to ensure that you understand your condition and your treatment options, as well as we wish to empower you to make changes needed to help you live a healthier life and manage your conditions.
Finally, we ask that if you at anytime have any problems or concerns that you let us know. Our goal is to put you our patient first and to serve you the best we can, and if we are not meeting that expectation just let us know what we can do to change.
