Insurance And Office Payment
Our practice participates in many insurance plans. Since each plan has different requirements, coverage limitations, and exclusions,
it is the responsibility of the patient
to understand and meet the requirements of their individual plan.
Prior to your visit
* Check to see if your insurance needs a referral to see a specialist (ophthalmologist)
* Check you vision benefits; some vision benefits are not taken by the office while others require you to have a 'routine eye exam' only
* Check for separate vision plans that are not part of your medical plan. Some require prior verification.
*Check you deductible.
*Most insurances have a "co-pay"
Most insurances do not cover the refraction. This is a test where you are asked 'which is better..one or two', for eye glasses. With most insurances, this is not included in your co-payment. If your insurance does not cover the refraction fee, we will collect this along with any co-payments at the time of your visit.
We accept payment by cash, personal check, Visa, MasterCard, Discover and American Express.
