Transferring of Records

You will need to request in writing if you want to have copies of your records sent to another doctor or organization. You will need to include the specific dates of service, medical information requested and the reason for this request, please include your date of birth and social security number on your request. If you wish to obtain a complete copy of the medical yourself, there will be a charge of $1.00 per page. Otherwise, there is no fee to sent records to another physician.
