About Our Office
We do not accept Insurance. You must upload a credit card with all your information in order to make an appointment. We also require you to fill out your entire history form within 48 hours of your scheduled appointment, otherwise the appointment will be canceled. We seek motivated clients that desire the upmost attention during their visit with us.
In order to create an exceptional clinical experience, our Nurse Practitioner, will review your medical records, make notes and be ready well in advance of your visit. Therefore, the minute you walk in to our office, we can give you the utmost attention and provide an outstanding personal clinical experience.
Because we are such a small practice with a limited number of appointments available, if you choose to cancel your appointment, you must do so within 24 hours of your scheduled appointment time otherwise you will be charged for the entire visit even if you do not attend. We desire to create true partnerships with our patients. We realize we may not be for everyone, but appreciate your understanding regarding these matters.
