Hello and welcome to our patient portal. 
  
Please take time to review my office policies prior to requesting an appointment. 

 

We offer virtual appointments Monday - Thursday with the option to have in-person appointments on Tuesdays and Wednesdays.  Effective May 11th, 2023 the federal Covid-19 Public Health Emergency (PHE) will end and all appointments will be conducted in-person or virtually via Google Meet. 

I do not participate in any insurance plans. This means all visits will be considered self-pay only.  You may be eligible to file for reimbursement of your out-of-pocket costs if you have a PPO plan. I am happy to provide a "Superbill" for you to submit to your insurance plan. If you have a Flexible Spending Account (FSA), you may request a receipt to file for reimbursement according to the guidelines set forth by your employer.
