Patients new to Riviera ENT, even if they have seen one of the physicians before, will need to have their prior records sent to Riviera ENT.  The MEDICAL RECORD RELEASE FORM can be uploaded on this portal, or it can also be found on https://www.rivieraent.com/current-future-patients  and can be completed and emailed to info@rivieraent.com or faxed to (805)679-5183.

Patients new to Riviera ENT, even if they have seen one of the physicians before, will need to either fill out all information available on this portal, or complete the NEW PATIENT COMPLETE INFORMATION PACKET found on https://www.rivieraent.com/current-future-patients  and can be completed and emailed to info@rivieraent.com or faxed to (805)679-5183.

Patients new to Riviera ENT who have completely filled out the portal will still need to fill out a single page NEW PATIENT CURRENT PROBLEM found on https://www.rivieraent.com/current-future-patients  and can be completed and emailed to info@rivieraent.com or faxed to (805)679-5183 or brought to the appointment.

Patients who are returning to Riviera ENT will need to fill out the RETURN PATIENT UPDATE FORM available on this portal or at https://www.rivieraent.com/current-future-patients and can be completed and emailed to info@rivieraent.com or faxed to (805)679-5183 or simply brought to the appointment.
