Transferring of Records

The patiet will need to sign a request to have copies of medical records sent to another doctor. There will be a charge for records released to an individual. Total cost will be determined by the number of pages to be copied. Payment of records must be made prior to record release to the patient/patient representative. Payment may be made in the form of cash or credit/debit cards.
