New Patient Instructions
Verify and Update Medical, Personal and Additional Information
Expand the
Medical Information Tab
and complete each section that relates to you.
** To add your medications, Click on the "Medications" tab.Search for the medication by name. Options will dropdown. Click on the green "+" in the row with the name ONLY (not add). The medication with different dosage options will dropdown. Click on the green "+" of the appropriate dosage and it will be added to your list.
Expand the
Personal Information Tab
and review the information entered by our office staff and update if needed. Main contact phone number must be listed as the Home Phone (even if it is a cell number).
**If you have a change of insurance, please notify the office and they will make that change for you to ensure claims are submitted correctly.
Expand the
Additional Information Tab
Click on any item and you will receive a pop-up message stating that you will be redirected
to a new tab. Click Continue.
