Patient portal Instructions/Information
The Home page contains the following:
Medical Information Tab
By expanding this tab, you can add/update Medical Information (medical conditions, medications, pharmacies, medical, social, and family history).
** To add/update the "Medications" tab. Search for the medication by name. Options will dropdown. Click on the green "+" in the row with the name ONLY (not add). The medication with different dosage options will drop down. Click on the green "+" of the appropriate dosage and it will be added to your list.
Personal Information Tab
By expanding this tab, you can update your personal/contact information. Please review the information entered by our office staff and update if needed. Main contact phone number must be listed as the Home Phone (even if it is a cell number).
**If you have a change of insurance, please notify the office and they will make that change for you to ensure claims are submitted correctly.
Additional Information Tab
By expanding this tab, you will see a list of items. They are:
-access appointment history,
-book appointments*,
-request appointments*,
-view your clinical summary,
-view test results*,
-get your patient profile printout,
-view educational content
*Based on practice settings
Click on any item and you will receive a pop-up message saying that you will be redirected
to a new tab where you can complete any of the above actions and more. Click Continue

Manage My Medical Record
This page gives you access to the same functions as the Home page as well as
various additional tasks such as request appointments, request refills, submit billing questions, upload documents to the office, send secure emails to the office and view your inbox.
Phone and Contact Icon
– You can add/remove/update your emergency contact information. Please be sure an emergency contact is entered.
Directives Icon
– allows you to review and sign consent forms, office policies, etc.
Additional Info shared with Practice Icon
– allows you to upload and submit information to the office.
My Healthcare Providers Icon
– allows you to search and add your primary care doctor, therapist or other specialist you wish to have on file.
